My name is Kimberly Michelle Miller. I was born in U of P on December 21 1996 which was a Saturday. My parants named me Kimberly because they like the name. My 9th grade year was bad for me I had my ups and downs I all ways got in to trouble and did not lesson to no one. I was my own self. My 10th grade year a got a personal care assistant and that year I did pretty good I run in t my best friend and then that year went good. I did not get in to fight seen my best friend was there I was proud of my self till the end of the year came around I got back in to my bad ways again and did not like it not even my best friend could help me at that point I just did not care about nothing. then my 11 grade I did a lot more better I got in to one fight and none after that I had a good year. AS I was in 11th grade I know this kid name mike and he try to get me in to tribal and he did not like me and I did not like him. I am in my 12th grade year now and I am good I have a best friend name Julia and Caitlin and I have a boyfriend name jay and I love them all every much. On June 5 2015 it is Senior Appreciation day. I have attended upper Darby high school since my freshman year. prior to attending upper Darby high school, I attended the Elwyn Philadelphia preschool Early intervention program and ST. Francis De sales School. I visits the emotional support resource room on a daily basis. I reports here any time there is a substitute in my classes, sometimes for tests, during grade room study hall, lunch , and before I leaves for the day. I user this room for emotional support when I am upset, for academic support for tests and quizzes, and at times for an escape when I want to avoid academic demands. Throughout my high school careers, I went from a freshman student who used to try and avoid any and all of my classes to now, a senior who attends the majority of my classes each day. I have been making progress in participating in my classes each year and weaning the emotional support resource room out of my schedule. This year, I started out trying to avoid my classes and hid in the resource room but after several small interventions with the emotional support teacher and the curriculum teachers, I have been able to maintain sufficient attendance in my classes during the 1st quarter. While in the resource room, I would either complete work or sit quietly until the bell rang. I will attend a vocational or technical school after graduating. I will meet with the guidance counselor and discuss appropriate courses and post secondary options. I involved in a specific learning and emotional support program and will work on specific weaknesses in reading fluency and reading comprehension math and written. I will seek employment in the field of computer technology with support. I will visit the career center to discuss and learn about employment option based on interests and levels. I will access community resources independently with support. I will meet administrative teacher and discuss progress toward obtaining the skills to be able to live independently. I will follow the designated routines and steps for making a successful transition from one activity period to another, with no more than 1 verbal or nonverbal prompt, in 5 of 5 random observations, for 3 consecutive marking periods. I will communicate with my personal care assistant. I will communicate my needs and ask for assistance when needed. I will develop a new routine when transitioning between my second semester classes. My PCA, Personal care assistant, should meet me at my bus stop at 6:30am and ride bus 60 with me to school each day. upon arrival to udhs, the PCA should walk with and supervise me in the hallways and in other unstructured environments, outside of the classroom, throughout my day. I am involved with a conflict or argument with another student. I am threatened by another student. I respond with verbal and or physical threat. I respond physically to the other student. Immediate tier 2 write up, leading detention or suspension. To gain power feeling of control. To avoid escape, or postpone Frustrating situation, class work, or other challenging work.

What I been doing Seens I got out of school
I live with my mom, dad, brother, sister, brothers girlfriend, and 3 dogs. I have problems seeing in my right eye the only thing I can see is light, I also have a bone missing in my right shoulder, I also fracsherd my left elbow, and my left rist, I sprat my right rist, I have mess up knees because I got pushed down the stairs. If I am not how I would be at my cousin house I spend a lot of time there.

I have very poor vision in my right. The vision is not expected to improve. I must wear protective eye wear to protect her right eye at all times. With regards to physical education class I should not participate in sports that involve a projectile or any contact sports where I might get hit in the eyes. I can participate in more general exercise classes as well as weight lifting training classes. Again I must wear protective eye wear. My glasses suffice as they are made with polycarbonate lenses. Sports goggle would also be acceptable. I had previous eyelid surgery as a child and now has bilateral corneal scarring which appears inactive at this time.

I am a 18 year old young woman who is seeking help from ovr with job training and development. I feel I have a particular strength in my ability to work with computer software and Internet related problems and would optimally like to get a job that uses that strength. I was in special education through grade and middle school with a mix of regular and special education classes in high school. I did well in regular classes. I was not allowed to go to votech however because my reading and math scores were so low. I had not worked since graduating this past June and spends my time at home watching tv and doing homework. I said I was not able to work because of not being able to see out of my right eye. I am the youngest of four children although my oldest brother has passed. I said I am close to my father and older brother. I said there were no unusual issues or problems within my family. I have experienced bullying in school and appears to have had difficulties with friendships perhaps as a consequence. I said I had one good friend at present. I said I was also limited because I was told by the high school administration that I could not take part in any after school programs because of my vision problem.
